Value: The total CIHR, SSHRC and SPOR amount available for this funding opportunity is $24,000,000, enough to fund approximately twelve (12) grants. The maximum amount per grant is $400,000 per year for up to five (5) years, for a total of $2,000,000 per grant.
Description: Transforming Health with Integrated Care (THINC) Implementation Science Team (IST) Grants focus on improving our understanding of how to implement, evaluate, and spread/scale (share) transformative evidence-informed integrated care policies and interventions towards advancing the Quadruple Aim and health equity (sometimes known as the Quintuple Aim). This funding opportunity will support research projects aimed to improve the implementation, evaluation, adaptation and/or spread/scale (share) of evidence-informed integrated care policies and interventions that advance any/all of the Quadruple Aim goals and health equity for one or more priority populations.
Priority populations include people with complex health needs; those currently or at risk of experiencing poor health outcomes based on social economic factors and/or those identifying with historically underrepresented populations. This funding opportunity will support projects relevant to the following research areas:
· Health Services and Policy
· Rural, Remote and Northern Communities
· Indigenous Integrated Care
· Aging in the Right Place
· Gender-Affirming Health
· Transforming Health and Well-Being for Children and Youth
· Genomics in Routine Care
· Care of Patients Covered within the IMHA mandate
· Multimorbidity
· Interdisciplinary Approaches to Equitable, Diverse and Inclusive Integrated Care
